Andrographis paniculata — known in Ayurvedic medicine as Kalmegh (“king of bitters”) and in Chinese medicine as Chuanxinlian — occupies a rare position in botanical pharmacology: it is both clinically validated by randomised controlled trials and mechanistically well-characterised at the molecular level. The plant’s primary bioactive, andrographolide, has been isolated, concentrated, and standardised to a degree that makes dose–response prediction possible — a prerequisite for rational clinical use that most herbal preparations cannot claim.
In integrative and functional medicine, andrographis is gaining renewed attention for three distinct applications: shortening and attenuating acute respiratory infections, modulating inflammatory pathways in chronic conditions including autoimmunity, and serving as a core herbal adjunct in chronic Lyme disease protocols. Each of these uses carries a different evidence profile, and distinguishing between them is essential for appropriate prescribing.
Mechanism: How Andrographolide Works
Andrographolide exerts its immunological effects primarily through covalent inhibition of NF-κB, one of the master transcription factors controlling the expression of pro-inflammatory cytokines including TNF-α, IL-1β, IL-6, and IL-8. Unlike NSAIDs, which block cyclooxygenase enzymes downstream in the inflammatory cascade, andrographolide acts upstream, reducing the transcriptional drive that initiates the cascade in the first place.
Mechanistically, andrographolide alkylates cysteine residues on the p50 subunit of NF-κB, preventing its nuclear translocation. This mechanism has been confirmed in cell-free systems and validated in human peripheral blood mononuclear cells (PBMCs) stimulated with lipopolysaccharide (LPS).
Secondary mechanisms include:
- Induction of haem oxygenase-1 (HO-1) — a cytoprotective enzyme with anti-oxidant and anti-apoptotic properties
- Activation of Nrf2 — the transcription factor that upregulates glutathione synthesis and detoxification pathways
- Inhibition of platelet-activating factor (PAF) — relevant to endothelial inflammation and microclot formation
- Stimulation of NK cell activity and macrophage phagocytosis — the immune-activating dimension that complements its anti-inflammatory effects
This dual profile — immune activation plus controlled anti-inflammatory signalling — distinguishes andrographis from purely immunostimulatory herbs (such as Echinacea) and from purely suppressive agents.
Clinical Evidence: Respiratory Infections
The best-supported indication for andrographis is uncomplicated upper respiratory tract infection (URTI). A systematic review by Coon and Ernst published in Planta Medica (2004; PMID 15095142) evaluated seven randomised controlled trials and concluded that andrographis extract was statistically superior to placebo in reducing symptom severity and duration, with a safety profile comparable to placebo. The effect size was clinically meaningful — reduction in symptom scores of 30–50% versus placebo — and the evidence quality met the bar for conventional Cochrane-style review.
The KalmCold RCT (Saxena et al., Phytomedicine, 2010; PMID 19692197) randomised 223 patients with acute URTI to either 200 mg KalmCold (standardised andrographis extract) or placebo for five days. The treatment group showed significant reduction in fatigue, throat symptoms, nasal discharge, and earache versus placebo, with no significant adverse effects. Time to recovery was shortened by approximately two days.
The Kan Jang preparation — a proprietary combination of andrographis and Siberian ginseng (Eleutherococcus senticosus) — has been evaluated in a double-blind trial involving 185 patients with sinusitis (Gabrielian et al., Phytomedicine, 2002; PMID 12487322). The combination outperformed both placebo and standard antibiotic treatment for symptom resolution, though the sinusitis evidence base remains smaller than the URTI data.
Practical implication: In a clinical setting, andrographis is a first-line herbal option for acute viral URTI. Starting within 36–48 hours of symptom onset appears to confer the greatest benefit. For patients who are not candidates for antivirals or who prefer non-pharmaceutical approaches, it represents a category with genuine level-II evidence.
Anti-Inflammatory and Autoimmune Applications
Beyond acute infection, andrographolide’s NF-κB inhibitory activity has generated interest in chronic inflammatory and autoimmune conditions. Preclinical models demonstrate reduction of joint inflammation in collagen-induced arthritis, attenuation of neuroinflammation in experimental autoimmune encephalomyelitis (EAE), and suppression of IL-17 signalling in colitis models.
A small pilot randomised trial in rheumatoid arthritis (Burgos et al., J Ethnopharmacol, 2001; PMID 11535371) showed that andrographolide treatment reduced tender and swollen joint counts and lowered CRP, though sample sizes were insufficient for definitive conclusions. Larger trials remain an unmet need.
In clinical practice, andrographis is most appropriately used as an adjunct anti-inflammatory in patients with:
- Post-infectious inflammatory syndromes — the period after acute Lyme, EBV reactivation, or COVID when ongoing inflammation perpetuates symptoms despite pathogen clearance
- Mild-moderate autoimmune conditions not requiring full immunosuppression — as an anti-inflammatory bridge during titration of prescription medication
- Metabolic inflammation — elevated hs-CRP without clear infectious or autoimmune aetiology
Caution with biologics: In patients already on TNF-alpha inhibitors, IL-6 inhibitors, or calcineurin inhibitors, the additive immunomodulatory effect of andrographis is theoretical but not quantified. I would not combine andrographis with biologics without direct specialist supervision.
Andrographis in Lyme Disease and Tick-Borne Infections
Andrographis has become a cornerstone of several integrative Lyme disease protocols, most notably the herbal protocol developed by herbalist Stephen Buhner. The rationale spans three interconnected mechanisms:
1. Cytokine Modulation in Lyme-Driven Neuroinflammation
Chronic Lyme disease — whether representing active persistent infection, post-infectious immune dysregulation, or a combination — is characterised by elevated IL-6, TNF-α, and IL-1β in cerebrospinal fluid and peripheral blood. Andrographolide’s NF-κB blockade directly targets this cytokine signature, providing theoretical benefit in Lyme-associated brain fog, fatigue, and pain amplification.
2. Antimicrobial and Spirochetal Effects
Several in vitro studies suggest andrographolide exhibits direct antimicrobial properties against a range of pathogens including Gram-positive bacteria and certain mycobacteria. Specific anti-Borrelia data in peer-reviewed literature is limited, but the broader antimicrobial portfolio, combined with biofilm-disrupting properties observed in some studies, supports its inclusion as an adjunct (not a monotherapy) in Lyme protocols.
3. Herxheimer Reaction Modulation
One of the more clinically interesting applications is using andrographis to reduce herxheimer reaction severity during antibiotic treatment. The anti-inflammatory properties may blunt the cytokine storm that accompanies bacterial die-off without substantially compromising antibiotic efficacy. This remains observational and protocol-based rather than RCT-proven, but it is mechanistically coherent and frequently reported in patient experience.
Typical Lyme adjunct protocol (as used clinically):
- Andrographis 400 mg standardised extract (10% andrographolide) twice daily
- Combined with other biofilm-disrupting agents and immune modulators
- Continued for 3–6 months depending on clinical trajectory
- Paused 1 week per month in some protocols to prevent tolerance
Patients should be counselled that andrographis is not a substitute for evidence-based antibiotic therapy where indicated. Its role is to modulate the inflammatory environment that perpetuates symptoms, not to eradicate Borrelia infection independently.
Dosing, Formulations, and Protocols
Standardisation
The most critical variable in andrographis supplementation is standardisation. The active dose is measured in milligrams of andrographolide, not in milligrams of total herb. Products should specify andrographolide content; formulations vary from 1% to 30% andrographolide by dry weight.

Absorption
Andrographolide has moderate oral bioavailability with a Tmax of approximately 1.5–2 hours. Food does not significantly alter absorption. Lipid-based formulations and phytosome complexes (andrographolide bound to phosphatidylcholine) improve bioavailability and may reduce gastric irritation.
Cycling
For long-term use, periodic cycling — four to six weeks on, one week off — is conventional clinical practice to prevent receptor desensitisation of NF-κB pathway regulation and to allow reassessment of baseline symptoms.
Safety, Contraindications, and Drug Interactions
Absolute Contraindications
- Pregnancy: Andrographolide has demonstrated uterotonic and abortifacient properties in animal studies. It is contraindicated in pregnancy without exception.
- Lactation: Insufficient safety data; avoid as a precaution.
Relative Contraindications (Requires Clinical Judgement)
- Autoimmune disease on immunosuppression: Immunostimulatory dimension may theoretically antagonise calcineurin inhibitors (ciclosporin, tacrolimus). Monitor if combined.
- Active hepatitis: Paradoxically, while andrographis has hepatoprotective properties in some contexts, high doses may cause transaminase elevation in pre-existing liver disease.
- Pre-operative: Discontinue 2 weeks before surgery due to anti-platelet effects.

Adverse Effects

- Rare: Urticaria or allergic reactions in sensitive individuals
Severe hepatotoxicity has been reported in isolated case reports at very high doses; standard therapeutic doses with appropriate standardisation do not carry this risk.
